About The Position

The Licensed Social Worker supports residents’ social and emotional well-being and provides assistance to residents and families. The role requires reliability, trustworthiness, consistent attendance, the ability to multitask and remain composed in a dynamic environment, and respectful collaboration with others.

The Social Worker develops resident social histories and assessments, creates individualized care plans, participates in interdisciplinary care planning, and provides individual and group support during crises or transitions. The position also assists with resident orientation and adjustment to the facility, coordinates discharge planning, facilitates staff training related to social services, and builds relationships with external partners to support referrals and ongoing care.

Requirements

• Bachelor’s Degree in Social Work (BSW), or certification as a Social Worker Designee with relevant secondary education.
• Effective communication and interpersonal skills.
• Experience working in long-term care settings.
• Compassion, organization, and commitment to residents’ holistic well-being.
• Reliability, trustworthiness, consistent attendance, ability to multitask and remain composed in a dynamic environment, and ability to work collaboratively and respectfully.

Responsibilities

• Develop detailed social histories and assessments for residents, identifying medically related social and emotional needs.
• Create individualized care plans that support resident goals and emotional well-being.
• Participate in interdisciplinary care plan conferences and contribute to overall care planning.
• Provide individual and group support to residents and families during times of crisis or transition.
• Assist with new resident orientation and help residents and families adjust to the facility.
• Coordinate discharge planning to support smooth resident transitions.
• Facilitate staff in-service training and educational programs related to social services.
• Build relationships with hospitals, doctors, churches, and community agencies to support referrals and ongoing care.
