About The Position

Pathways PACE has an immediate opening for a Social Worker, required LSW or LCSW licensure. This role provides services to meet the social and/or emotional needs that affect the participants’ ability to achieve their goals; participate in the development and implementation of participant comprehensive care plans. Ensures compliance with regulations, professional licensing, and standards of practice. The organization is a network of mission-driven, nonprofit healthcare organizations dedicated to caring for very ill, frail, and primarily elderly individuals by keeping them at home and out of the hospital while maximizing comfort, dignity, and quality of life.

Requirements

• LSW or LCSW licensure required.
• Master’s degree in Social Work required.
• One year experience working with older adults.
• Knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling.
• Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute.
• Valid BLS Certification.

• Three to five years of professional social work experience in health care or mental health preferred.

Responsibilities

• Provide services to meet the social and/or emotional needs that affect the participants’ ability to achieve their goals.
• Participate in the development and implementation of participant comprehensive care plans.
• Ensure compliance with regulations, professional licensing, and standards of practice.
• Demonstrate a working knowledge of the stages and manifestations of change, loss, and grief, including complicated grief.
• Recognize signs and symptoms of psychiatric illnesses.
• Identify physical, behavioral, and affect changes to be reported to the attending physician or other team members.
• Demonstrate critical thinking skills in discriminating between protection of sensitive medical/psychosocial data and compliance with applicable law and regulation regarding mandated reporting.
• Establish therapeutic relationships and maintain healthy interpersonal boundaries with participants and families from a variety of cultural and socioeconomic backgrounds.
• Demonstrate good psychosocial skills of participants from various age groups, including a working knowledge of cognitive stages, growth and development theory and manifestations.
• Demonstrate professional counseling skills encompassing but not limited to engaging participants and caregivers, reframing, reflecting, clarifying, affirming strengths, normalizing emotions, setting goals and limits, and involving participants in the plan of care.
• Identify the need for consultation with other members of the interdisciplinary team or with experts in specialized areas of psychosocial practice.
• Demonstrate the ability to access various community resources.
• Assess on an ongoing basis the psychosocial needs of participants and families based on emotional, environmental, financial, psychological, cognitive, social, cultural, and health factors.
• Assess the impact of altered health status on the ability of the participant and family to employ adaptive coping mechanisms and manage the care of the participant.
• Initiate referrals to appropriate community resources and assist the participant and family in accessing resources.
• Assist the participant/family in applying for Medicaid benefits and other financial support.
• Collaborate with the interdisciplinary team in the development, evaluation, and revision of the plan for psychosocial care of the participant/family unit.
• Involve the participant/family in determining care goals and objectives.
• Implement the plan of care such that goals and objectives are met.
• Complete reassessments of systems identified in the plan of care for ongoing monitoring and as warranted by participant needs.
• Provide short-term individual counseling to participants and families.
• Document care provided and the response of the participant/family in a risk-neutral fashion; recognize the sensitivity of psychosocial data and protect confidentiality of same; submit documentation in a timely manner that thoroughly reflects the intervention and care provided by the social worker and meets agency policy.
• Participate in the Counselor On-Call rotation.
• Model attendance at Colorado PACE events and in-services.
• Develop partnerships within the community and educate community members and healthcare providers about Midland services.
• Provide education related to Midland programs in alternate care settings (nursing homes, hospitals, assisted-living facilities, etc.).
• Monitor and respond to reports including Encounters, Attendance, QA, Satisfaction Surveys, Concern Reports, and Budget.
