Licensed Social Worker
About The Position
We are seeking per diem Social Workers to work the following shifts: Weekdays from 5:00pm to 8:00pm, weekends as needed. We are seeking qualified Licensed Social Work Professionals to join our team in our Nursing Home. We want to speak with naturally caring, empathetic Social Workers who are focused on providing the best quality of life possible for every patient in their care with patience, courteousness and tactful interactions with residents, staff, family members and visitors. Reporting to the Director of Social Work, Social Workers at Parker deliver quality social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for coordinating the discharge planning process, participating in interdisciplinary team meetings and quality improvement activities, as well as counseling and meeting the satisfaction of the patients, residents and family members. The Parker Jewish Institute for Health Care and Rehabilitation, conveniently located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility that offers inpatient programs such as sub-acute/short term rehabilitation, long-term care and nursing home care, as well as community health services encompassing certified home health care and a comprehensive community hospice program that serves terminally ill patients in their own homes or in nursing facilities, including Parker's nursing home. Requirements
- Master's Degree in Social Work
- Active licensure in NYS
- Prior experience in a long-term care, rehabilitation or hospital setting
- Strong communication skills
- Ability to work collaboratively across a variety of disciplines
- Familiarity working with EMR (electronic medical records)
