Licensed Social Worker
About The Position
This is a full-time position for a Licensed Social Worker in Little Rock, Arkansas. The role involves maintaining and improving residents' ability to manage their daily physical and mental/psychosocial needs. Key responsibilities include identifying social and emotional needs, developing individualized care plans, and participating in care plan meetings. The social worker will also collaborate with the finance team regarding resident documentation and assist with discharge planning to ensure residents are safely reintegrated into the community with necessary support like Durable Medical Equipment (DME) or home care.
Requirements
- At least one year experience in Social Work field required
Nice To Haves
- BSW or MSW preferred, or Bachelors degree in related field
- Experience in long term care and/or sub-acute setting preferred
Responsibilities
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
