Requirements
- A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation or be a licensed Social Worker
- 1 year of supervised social work experience in healthcare
Responsibilities
- Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
- Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
- Complete assessments and participate in comprehensive care plans.
- Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.
- Maintain confidentiality of protected health information, including verbal, written and electronic communications.
- Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.
