Licensed Social Worker
About The Position
The Licensed Social Worker (LSW) oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards. The LSW works directly with discharge planners, family or responsible party to initiate a tour of the facility and follow up procedures. The role involves assisting in the development and implementation of policies and procedures for identifying the medically-related social and emotional needs of the patient/resident, and maintaining good working rapport with intra-department personnel, other departments, and outside community health, welfare, and social agencies. The LSW must stay informed about current federal and state regulations and professional standards.
Requirements
- Must have a current Social Work license, in good standing with the state (as required)
- Must have at least 3+ years of experience in a long term care facility
- Must be able to organize and prioritize many tasks effectively
- Ideal candidate must be a skilled communicator, director and motivator
- Reliable transportation required
Nice To Haves
- Bachelor’s Degree in Social Work strongly preferred
Responsibilities

- Develop and manage the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
- Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident
- Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents
- Keep abreast of current federal and state regulations as well as professional standards
