About The Position

Bring your passion for social work into the homes of families with high-risk infants, providing personal support and guidance through the Following Baby Back Home (FBBH) program. The Home Visiting Social Worker provides home visiting services to families within an assigned catchment area, partnering with the Clinical Nurse Coordinator to deliver coordinated care, caregiver education, and supportive services. This role helps families strengthen positive parent-child relationships, support their infant’s development, and connect with community resources. The position requires flexibility in daily and weekly scheduling to meet family needs, including some evening and weekend hours. A valid Arkansas driver’s license, navigational skills, and extensive travel throughout the state are required.

Requirements

  • Bachelor’s degree in Social Work.
  • Current Arkansas Social Work License at the Licensed Social Worker (LSW) level.
  • Valid Arkansas driver’s license.
  • Navigational skills and the ability to travel extensively throughout Arkansas.
  • Flexibility to adjust daily and weekly schedules to meet family needs, including some evening and weekend hours.
  • Proof of legal authority to work in the United States on the first day of employment.

Nice To Haves

  • Bilingual proficiency in English and Spanish.
  • At least one year of experience in home visiting.

Responsibilities

  • Conduct home visits and provide age-specific, evidence-based education on child development, positive parent-child interactions, parenting concerns, safety, and community resources.
  • Adjust visit schedules to meet family needs, including after regular working hours.
  • Complete psychosocial, developmental, and other required assessments.
  • Partner with the Clinical Nurse Coordinator and families to develop individualized, family-centered care plans.
  • Connect families with mental health services, socioeconomic resources, and early intervention programs.
  • Document significant contacts and actions according to FBBH policies, and follow up with families according to care plans and program standards.
  • Collaborate with healthcare providers, community agencies, and program leadership to coordinate services and maintain positive working relationships.
  • Follow administrative directives, business practices, attendance policies, and professional and institutional ethical standards.
  • Apply Family-Centered Care principles in all duties.
  • Complete 30 hours of professional development every two years, plus annual and program-required training.
  • Perform other duties as assigned.
