About The Position

Attain Behavioral Health is seeking a Group Facilitator to lead engaging, person-centered Therapeutic Behavioral Services (TBS) groups within partner nursing homes. The role combines mental health support, therapeutic engagement, and relationship-building with residents who benefit from structured emotional and behavioral health programming. The position is based in off-site facilities in Sandusky, Ohio, and involves working independently while collaborating with facility staff and the on-site clinical team.

Requirements

Hold an LSW, LPC, or equivalent Ohio-recognized behavioral health credential.
Have experience facilitating behavioral health or substance-use groups.
Have strong communication skills, empathy, and the ability to connect patiently and respectfully with residents.
Be able to work independently in off-site facilities.
Have reliable transportation.
Meet Ohio Medicaid and background check requirements.

Nice To Haves

Experience working with older adults, in long-term care, or with medically complex populations.
Experience with EHR systems; training is available.
Trauma-Informed Care training, or willingness to obtain it within 90 days.
Comfort working in nursing home environments and ability to walk or stand as needed.

Responsibilities

Facilitate TBS groups that support residents’ emotional health, coping skills, motivation, and quality of life.
Adapt group content to the cognitive and emotional needs of older adults, including residents with medical complexities or mild cognitive impairment.
Collaborate with facility staff and the on-site clinical team to provide consistent, compassionate care.
Complete group notes, progress updates, and Medicaid paperwork accurately and on time.
Encourage resident participation, foster connection, and create a safe, respectful group environment.
Participate in ongoing supervision and professional development.
