About The Position

The Licensed Practical Nurse (LPN) provides licensed nursing services to residents in memory care, following Senior Lifestyle Corporation guidelines and applicable federal and state regulations. The role includes medication administration, resident health assessments and care planning, clinical documentation, and responding to emergencies. The LPN communicates with physicians, family members, and staff regarding residents’ health and supports resident and staff education.

Requirements

Training from an accredited licensed or vocational nursing program.
At least 1 year of experience in senior care or a related field.
Current Licensed Practical Nurse license in the state of practice.
CPR certification.
Acceptable proficiency in English to perform job duties, communicate with supervisors and other employees, and respond to and communicate about emergencies.
Basic computer skills.
Ability to work as part of a team and handle multiple tasks safely and effectively.
High tolerance for working under pressure and handling critical situations calmly and professionally.

Responsibilities

Administer medications to residents in compliance with federal and state regulations and Senior Lifestyle Corporation guidelines.
May admit new residents, including completing admission assessments and documentation.
Complete acute and long-term care plans or service plans.
May track clinical systems, such as wounds or infection control, under the director’s direction.
Provide general consultation regarding health-related concerns within the community.
Notify physicians and/or family members about changes in residents’ health and document appropriately.
Respond quickly to emergencies.
Maintain close communication with all departments and staff members.
Provide educational meetings for residents and staff.
Understand the role in the Safety & Disaster Plan.
Maintain confidentiality of residents’ and prospective residents’ information.
May complete discharges, assessments, and clinical documentation as needed.
