About The Position

The Licensed Practical Nurse provides licensed nursing services to residents in the memory care community, following Senior Lifestyle Corporation guidelines and applicable regulations. The role includes medication administration, resident assessments and care planning, health-related consultation, communication about changes in residents’ health, emergency response, and clinical documentation. The position is part time, with a 7 a.m.–7 p.m. schedule, at The Estate at Hyde Park.

Requirements

Training from an accredited licensed or vocational nursing program.
General nursing area of study.
At least 1 year of experience in senior care or a related field.
Current Licensed Practical Nurse license in the state of practice.
CPR certification.
Acceptable proficiency in English to perform job duties, communicate with supervisors and other employees, and respond and communicate in emergency situations.
Basic computer skills.
Ability to work as part of a team and handle multiple tasks safely and effectively.
High tolerance for working under pressure and handling critical situations calmly and professionally.

Responsibilities

Administer medications to residents in compliance with Federal and State regulations and Senior Lifestyle Corporation guidelines.
May handle admissions of new residents, including admission assessments and documentation.
Complete acute and long-term care plans or service plans.
May track clinical systems, such as wounds or infection control, under the director’s direction.
Provide general consultation about health-related concerns within the community.
Notify physicians and/or family members of changes in residents’ health and document appropriately.
Respond quickly to emergencies.
Maintain close communication with all departments and staff members.
Provide educational meetings for residents and staff.
Understand the role in the Safety & Disaster Plan.
Maintain confidentiality of residents’ and prospective residents’ information.
May handle discharges, assessments, and clinical documentation as needed.
