About The Position

Pinnacle Home Care is hiring a Licensed Practical Nurse (LPN) to deliver compassionate, patient-centered home health care. The role provides skilled nursing care under RN supervision and supports patients’ healing and independence through individualized care in their homes. The position includes documenting care in an electronic medical record, coordinating schedules, and following agency policies and quality programs. This is a PRN role with flexible scheduling.

Requirements

• Current state-specific LPN or LVN license.
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience; home health or medical/surgical experience is preferred.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks according to patient care plans.
• Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
• Complete visit reports and required documentation promptly and accurately in the electronic medical record system.
• Confirm daily visit schedules to coordinate care with other staff.
• Understand the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with service agreements and admission policies.
• Follow agency policies regarding appropriate patient discharge and reasons for discharge.
