About The Position

Avalon Village is hiring a Licensed Practical Nurse (LPN) for full-time evening and night shifts. The nurse supports residents’ physical and psychosocial well-being and builds meaningful relationships by learning about residents’ and staff members’ life stories. The role involves providing medication management, assessing residents, documenting changes in their condition, and working with nursing and other facility teams to deliver coordinated care. Requirements

Hold a current, valid Licensed Practical Nurse license in Indiana.
Have proficient medication management skills.
Be able to conduct thorough assessments and accurately document changes in resident condition.
Have a strong passion for geriatric nursing and a commitment to senior care excellence.
Have excellent communication and interpersonal skills.
Demonstrate supportive presence, leadership, collaboration, teamwork, and interpersonal communication skills.

Responsibilities

Create a comforting and engaging atmosphere for residents.
Promote teamwork within the care team to meet residents’ needs.
Collaborate with nursing and other facility teams to ensure coordinated, comprehensive care.
Provide medication management.
Conduct thorough resident assessments and accurately document changes in resident condition.
