Licensed Practical Nurse Psych Visits PRN
About The Position
Elara Caring is hiring a Licensed Practical Nurse for behavioral health visits in the Bridgeport, Stratford, and Trumbull areas. This PRN role provides care in patients’ homes and offers an opportunity to support patients and caregivers through compassionate, coordinated care. The nurse follows individualized care plans and physician orders, monitors patient progress, supports self-care education, and works with the RN Case Manager and the broader care team to adapt care as needs change. The position also involves documenting medication regimens, communicating with physicians, patients, and families, and helping prevent avoidable hospitalizations and emergency room visits. The stated base salary is $30.26 to $45.00 per hour. Actual pay is determined by factors including education, experience, skills, and internal equity. Additional compensation may be available, including performance-based bonuses, commissions, on-call pay, or other incentives; these are not guaranteed. Full compensation and applicable benefits are discussed during the interview process.
Requirements
- Current state license as a Licensed Practical Nurse.
- Graduate of an accredited certificate, diploma, or associate school of nursing.
- One year of experience in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities
- Follow each patient's individualized care plan and physician orders.
- Monitor patient progress and report changes or safety concerns to the care team.
- Help patients and caregivers learn self-care techniques to support their health.
- Work with the RN Case Manager to update care plans as patient needs change.
- Document medication regimens and update medication profiles according to agency guidelines.
- Communicate with physicians, patients, families, and the care team to ensure coordinated care.
- Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
