About The Position

Adoration Home Health and Hospice is seeking a dedicated Home Health Licensed Practical Nurse to provide patient-centered care in Albemarle, North Carolina, covering Stanly County and parts of Davidson and Montgomery counties. The role provides one-to-one care in patients’ homes, following each client’s plan of care and supporting their dignity and independence. The position is full-time and offers flexible scheduling options, independent work with team support, and opportunities for advancement.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted nursing registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide one-to-one care in accordance with each client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed, and assess and report results according to the plan of care.
• Accurately document nursing actions, care provided, communications, progress, and outcomes related to established goals.
• Inform the physician, case manager, and families about changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
