About The Position

The Licensed Practical Nurse (LPN) provides basic medical attention and assesses residents’ wellbeing as part of a larger medical team. The role calls for patience, compassion, and a gentle, empathetic approach to caring for elderly residents. The LPN coordinates nursing care on the unit, provides medication-related interventions and treatments, conducts nursing assessments, and communicates changes in residents’ health status and physician orders.

Requirements

• Current state license to practice as a Licensed Practical Nurse (LPN).
• Knowledge of basic nursing practice and an interest in gerontological nursing.
• Patience, compassion, and a gentle, empathetic approach to caring for elderly residents.

Nice To Haves

• Current CPR certification and additional certification in a nursing specialty.
• Two years of experience in a long-term care setting.
• Basic computer skills and experience with Electronic Medical Records.

Responsibilities

• Coordinate nursing care on the unit and ensure each resident is assigned a nurse aide capable of providing their care.
• Exercise independent judgment and discretion in performing the role.
• Provide safe and accurate medication-related interventions.
• Administer and document medications and treatments according to residents’ schedules and current medication-pass standards.
• Understand the rationale and effects of medications and treatments and administer them correctly.
• Provide physician-ordered treatments, including wound care, urinary catheterization, oxygen care, and tracheostomy care.
• Order and receive medications from the pharmacy.
• Account for narcotics and sign for counts and exchanges of medication-cart and medication-room keys.
• Respond promptly to residents’ requests for pain intervention and document interventions in resident records.
• Transcribe physician orders accurately and coordinate their implementation.
• Assess residents’ health status and notify physicians of changes, promptly implementing and documenting new orders.
• Prepare work assignment sheets while considering resident safety and staff members’ preparation, experience, knowledge, and ability.
• Report staff shortages to nursing management.
• Participate in and communicate employee actions that may result in reward or discipline.
