About The Position

Licensed Practical Nurse (LPN) needed for a travel assignment in Utica, NY, at a skilled nursing facility. This is a temporary assignment requiring NY LPN licensure with LTC experience and proficiency in PointClickCare. The role involves providing licensed nursing care to residents on the LTC unit, administering medications, monitoring patient conditions, and collaborating with the interdisciplinary team. The assignment is for a 13-week contract with guaranteed hours per week, working the night shift on a 40-bed LTC unit.

Requirements

Active NY LPN license; Nursys license verification for active licenses
Current BLS certification
1+ year of experience in a LTC facility required
EMR experience with PointClickCare
Drivers license with address that matches demographics
RTO (time off) requests must be included at time of submission
Willingness to travel to Utica, NY for the assignment

Responsibilities

Provide licensed nursing care to residents on the LTC unit, including assessments, monitoring, and interventions per the care plan
Administer prescribed medications and document administration in PointClickCare EMR
Observe, chart, and report changes in patient condition to the supervising RN; respond to urgent situations per facility protocol
Support activities of daily living, safety precautions, and appropriate patient positioning; assist with mobility and transfers as trained
Collaborate with the interdisciplinary team; participate in care planning and care plan updates; float within the same building as needed
Maintain accuracy and timeliness of charting, ensure a clean, safe, and respectful environment for residents
