About The Position

Mount Carmel Care Center is seeking a Licensed Practical Nurse (LPN) to assist the Unit/Shift Supervisor in planning, organizing, developing, and directing nursing unit activities. The role includes supporting resident assessments and care planning, monitoring residents’ conditions, documenting changes, and administering prescribed medications and treatments. This is a mission-oriented role at a faith-based, non-profit, 69-bed long-term care facility in the Berkshires. Full-time and part-time schedules are available, with all shifts open.

Requirements

Graduate of an accredited nursing program.
Current LPN license in Massachusetts.
BLS certification.
Must be able to supervise and delegate.

Nice To Haves

One year of nursing home experience preferred.

Responsibilities

Assist the Unit/Shift Supervisor in planning, organizing, developing, and directing nursing unit activities.
Assist the Unit/Shift Supervisor in identifying problems and developing correction plans.
Check ill residents early in the shift, conduct periodic assessments, and report changes.
Assess resident care needs for the Minimum Data Set, Resident Assessment protocols, and interdisciplinary care plans.
Report changes in resident condition and document them in the medical record.
Administer and record prescribed medications and treatments.
