About The Position

CLFC Healthcare and Communications is hiring a per diem Licensed Practical Nurse to work onsite at Wayne County’s 192-bed county skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. LPNs run medications and treatments on their assigned unit while supported by onsite leadership. The role offers a self-selected schedule with no minimum hours, paid weekly at $38.00–$42.00 per hour. Experience determines the rate within the range; night and weekend shifts are paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Shifts include eight paid hours with a half-hour unpaid meal break, with 12-hour shifts available when offered. Open shifts are posted in the BRIDGE App for nurses to select and confirm. Orientation is paid at the employee’s rate. CLFC arranges and pays for fingerprinting, immunizations, screening, and license verification. The role includes reimbursement for New York license and CPR renewals, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is offered for bringing a nurse or aide onto the roster. Paid sick leave accrues under New York law, and CLFC professional liability insurance covers each shift. Shifts canceled by the facility two hours or less before they start are paid for four hours; shifts canceled with more notice are removed from the schedule. This is a W-2 hourly per diem position.
