About The Position

The Licensed Practical Nurse (LPN/LVN) provides direct patient care to people in an acute rehabilitation setting through assessments, nursing interventions, documentation, and interdisciplinary coordination. The role supports safe, high-quality care and patients’ recovery at Dignity Health East Valley Rehabilitation Hospital – Chandler, a 40-bed inpatient acute rehabilitation hospital. The position is full-time, working nights on a 3x12 schedule from 6 p.m. to 6 a.m., with rotating weekends.

Requirements

• Current state LPN/LVN license.
• Graduation from an accredited LPN/LVN program.
• CPR/BLS certification required.
• IV certification is highly preferred or must be obtained within six months.

Nice To Haves

• At least six months of Medical/Surgical experience in an acute care setting is preferred.
• IV certification is highly preferred.

Responsibilities

• Administer medications safely and accurately, accounting for patient age and clinical condition.
• Follow medication administration procedures to minimize medication errors.
• Assist the RN with initial assessments and patient reassessments according to policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Complete timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain, use appropriate pain management techniques, and educate patients and families.
• Perform patient care in a manner that promotes safety and reduces medical errors.
• Develop and implement individualized teaching plans based on patient and family learning needs.
• Perform other duties as assigned.
