About The Position

Pathways Opiate Treatment Program (OTP) is an accredited program specializing in Medication-Assisted Treatment (MAT) using methadone. Our comprehensive approach includes individual and group therapy, psycho-educational groups, individualized treatment planning, recovery coaching, harm reduction services, and ongoing assessments and referrals. We operate with a strong multidisciplinary team model, blending clinical and medical expertise to provide exceptional care. Our dedicated team works daily to save lives and reduce risks for individuals and communities affected by opioid use disorder. Requirements

  • Current RN or LPN license in the State of Connecticut required
  • Certification in CPR
  • Must have a valid driver’s license

Nice To Haves

  • Degree/Certification in nursing or in a related health care field preferred
  • No experience required

Responsibilities

  • Administer daily MAT (including methadone and buprenorphine) in accordance with physician/APRN orders and regulatory standards.
  • Prepare and dispense individual doses, ensuring accuracy and adherence to chain-of-custody procedures.
  • Safely transport and secure controlled medications between CHR OTP clinics, mobile clinics, and correctional facilities in compliance with policy and regulatory requirements.
  • Monitor patient response to treatment, including conducting assessments and observing for side effects or adverse reactions.
  • Conduct and monitor toxicology screens.
  • Maintain accurate and timely documentation in the electronic health record (EHR), consistent with program and agency standards.
  • Manage medication inventory, including ordering, restocking, and reconciliation, in compliance with CHR and regulatory requirements (DPH, DMHAS, DEA, Joint Commission).
  • Communicate effectively with prescribers, clinical team members, and DOC staff regarding patient care, progress, and safety issues.
  • Participate in team meetings, supervision, trainings, and other required activities.
  • Ensure compliance with all DOC, CHR, and regulatory safety/security protocols.
  • Perform other related duties as assigned.
  • Per diem LPNs/RNs are required to provide coverage for at least one (1) weekend per month and two (2) holidays per calendar year.
