About The Position

Elite Care Management is hiring a Licensed Practical Nurse to provide one-on-one home health care to a single patient for the duration of each shift. The nurse gets to know the patient and their family, learns about the patient’s needs at home, and plans, organizes, and assists with home health care services. The role includes supporting patients’ daily living activities, comfort, overall well-being, and quality of life. The listed pay range is $35–$40 per hour, with a $5,000 full-time or $3,000 part-time sign-on bonus. Shifts listed include 6 a.m.–6 p.m. on Sunday and 6 a.m.–6 p.m. PRN on various weekdays and weekends.

Requirements

• Current RN or LPN state license.
• Current CPR card.
• Driver’s license.
• Social Security card.
• Auto insurance card.
• TB test results: two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.
• Strong spoken and written communication skills.
• Ability to manage and direct patient care according to a care plan and update it as needed.

Responsibilities

• Observe patients, record significant conditions and reactions, and notify a supervisor or physician of patient conditions, reactions to drugs or treatments, and significant incidents.
• Follow organizational and nursing policies and procedures.
• Communicate effectively in spoken and written form.
• Manage and direct patient care according to the patient care plan and update the plan as needed.
• Administer prescribed medications and treatments, and monitor and document their effectiveness.
• Respond as appropriate to changes in a patient’s condition and report changes to the appropriate physician or supervisor.
• Assist patients with daily living activities and support their comfort and overall well-being.
