About The Position
Select Home Health & Hospice, a clinician-led and operated skilled home health care agency serving central Indiana, is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide high-quality in-home nursing care. The role supports patients’ health, safety, and daily care needs as part of an interdisciplinary team. The position is located at Select Home Health in Carmel, Indiana. Requirements
- Graduate of an accredited nursing program.
- Active state LPN license in Indiana.
- Recent acute care or home health experience; home health experience preferred.
- Valid driver’s license, reliable vehicle, and auto insurance.
- Current TB test and CPR certification.
Nice To Haves
- Home health experience is preferred.
Responsibilities
- Provide skilled nursing care according to physician orders and care plans.
- Monitor patient condition, assess pain, and educate patients and families.
- Administer medications safely following the 7 rights.
- Ensure patient safety and maintain confidentiality.
- Communicate patient status and needs with the care team.
- Complete accurate and timely documentation.
- Participate in required trainings, in-services, and meetings.
- Follow agency policies, procedures, and emergency protocols.
