About The Position
Wellington Rehabilitation & Healthcare is seeking a compassionate, dedicated Licensed Practical Nurse (LPN) to provide high-quality care to residents in a skilled nursing and rehabilitation setting. The LPN will provide direct nursing care, conduct and document medical evaluations, assist with individualized treatment plans, and support resident safety programs. The role also includes supervising and supporting CNAs and performing additional duties as assigned to support resident care. Requirements
- Active Licensed Practical Nurse (LPN) license in good standing.

Responsibilities
- Assist in developing and implementing individualized treatment plans for residents.
- Supervise and support CNAs, including participating in their evaluations.
- Oversee the Falls Program and Pressure Sore Program to support resident safety and well-being.
- Provide essential nursing services and direct patient care.
- Conduct and document medical evaluations upon admission and throughout treatment.
- Perform additional duties assigned to support resident care.
