Licensed Practical Nurse (LPN)
About The Position
St. Elizabeth Home, an Illuminus Community, is hiring a Licensed Practical Nurse to provide prescribed medical treatment in a skilled nursing setting offering long-term care and short-term rehabilitation. The LPN works under the general supervision of a registered nurse and provides resident-centered, holistic care to older adults. St. Elizabeth is a not-for-profit, community-based organization serving people of all faiths in Janesville, Wisconsin. The position offers full-time, part-time, and PRN opportunities with varying scheduling options based on community needs. Shifts may be eight hours and may include a scheduled weekend rotation. Pay is $27.80 to $31.20 per hour, depending on experience.
Requirements
- Active Licensed Practical Nurse credentials and a license in good standing in the State of Wisconsin.
- Active CPR.
- Knowledge and/or experience in geriatrics.
Responsibilities
- Communicate professionally and effectively with residents, families or personal representatives, and staff, while promoting the organization's mission, vision, and values and serving as a professional role model.
- Provide on-call services as assigned.
- Develop, implement, and update plans of care, promote resident-centered care, and support residents in achieving and maintaining their highest practicable level of health and well-being.
- Administer and monitor medications, perform medical treatments and vital sign checks, and verify physician orders for accuracy and appropriateness.
- Lead, guide, and supervise CNAs and resident assistants in providing quality care.
- Assist with physician visits and provide staff feedback; ensure appropriate delegation, direction, and follow-through of assigned duties.
- Observe, document, and report resident status, including changes in condition and 24-hour reports.
- Promptly communicate concerns to the registered or charge nurse, family, or physician as appropriate, and provide a thorough transfer of information to the next team leader.
- Follow universal precautions and PPE requirements.
- Perform other duties as assigned.
