About The Position

Adoration Home Health is seeking a dedicated Licensed Practical Nurse to provide patient-centered home health care in Mobile, Alabama, with coverage in Baldwin County. The role provides one-to-one care in patients’ homes, following their plans of care while supporting their dignity and independence. The nurse works independently with team support and coordinates care with families, case managers, and physicians. The schedule is full-time, Monday through Friday.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least 1 year of nursing experience.
• Home health care experience.

Responsibilities

• Provide one-to-one care in accordance with each client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the plan of care, and assess and report results.
• Document nursing care, communications, progress, and outcomes for established goals; inform the physician, case manager, and family of changes in the client’s medical condition or needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
