About The Position

Hospice of the Western Reserve, now Reserve Care, is seeking a Licensed Practical Nurse (LPN) to join their team. This role operates on weekends, specifically Saturday and Sunday from 8:00 am to 8:30 pm, with a unique compensation structure where employees work 24 hours but are paid for 30. Reserve Care provides expert hospice, palliative, and grief support, emphasizing individual worth and compassionate care during life's important moments. The On-Call Nurse plays a crucial role in providing after-hours care to patients and families in urgent situations, offering skilled nursing services, pain and symptom management, emotional support, and acting as a liaison with physicians. This position offers the autonomy to utilize nursing skills and make a significant difference in the lives of others, often described as a calling rather than just a career.

Requirements

  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
  • Licensed RN in state of Ohio.
  • Experience preferred in pain and symptom management, oncology, or mental health.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.).
  • Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology.
  • Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR’s insurer.
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
  • Provide an active driver’s license.
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Responsibilities

  • Provide intermittent skilled nursing services to the patient under the patient’s physician’s direction.
  • Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
  • Serve as a liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer patients and/or family members to other services when appropriate.
  • Provide instruction (health care teaching) to the patient and family as related to care in a residence.
  • Make visit to patient’s residence at the time of death.
  • Maintain, promptly complete, and return all documentation as required.
  • Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by team leader.
  • Participates in mandatory or elective RC University events, trainings, or in-services as necessary.
  • Perform other duties as assigned.
