About The Position

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities. This role involves ensuring each individual receives quality, individualized health care management, conducting routine site visits, checking vitals, doing assessments, counseling on medication administration, documenting progress, acting as a health/medical resource for staff, training staff in the medical arena, and conducting audits and reviews of medications. The LPN will also ensure company compliance with state requirements and organizational policies regarding medical care and records. The position is full-time, Monday-Friday, with occasional travel required and flexible hours. It includes an on-call rotation for triage of medical concerns for one week per month, in a low-stress, relaxed environment. This is a case management role with semi-annual physical assessments, in a high morale, family-like, team-oriented environment.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Indiana required
  • 1-2 years experience working with individuals with disabilities or other long-term care is preferred
  • Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
  • Strong verbal/written communication skills
  • Ability to work independently and efficiently
  • Organizational and computer skills
  • Acceptable background check & driving record check

Nice To Haves

  • 1-2 years experience working with individuals with disabilities or other long-term care is preferred

Responsibilities
