We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities. This is a case management role with semi-annual physical assessments. It is a low stress, relaxed environment with high morale, a family-like, team-oriented environment.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Indiana required
  • Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
  • Strong verbal/written communication skills
  • Ability to work independently and efficiently
  • Organizational and computer skills
  • Acceptable background check & driving record check

Nice To Haves

  • 1-2 years experience working with individuals with disabilities or other long-term care is preferred

Responsibilities

  • Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
  • Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
  • Act as a health/medical resource for staff, training staff in the medical arena
  • Conduct audits and reviews of medications
  • Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
