About The Position

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in Bloomington, IN. This is a case management role with semi-annual physical assessments. The environment is described as low stress, relaxed, high morale, family-like, and team-oriented. The position involves ensuring individuals receive quality, individualized health care management, conducting routine site visits, checking vitals, doing assessments, counseling on medication administration, documenting progress, acting as a health/medical resource for staff, training staff in the medical arena, conducting audits and reviews of medications, and ensuring compliance with state requirements and organizational policies regarding medical care and records. Occasional travel may be required.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
• Acceptable background check & driving record check

Nice To Haves

• 1-2 years experience working with individuals with disabilities or other long-term care is preferred

Responsibilities

• Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
• Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
• Act as a health/medical resource for staff, training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
• Participate in an on-call rotation for triage of medical concerns for 1 week per month
