About The Position

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN. This is a case management role with semi-annual physical assessments, typically working Monday through Friday, 8am-5pm, in a low stress, relaxed, high morale, family-like, team-oriented environment. The role ensures that each individual receives quality, individualized health care management, resulting in their optimal health. Responsibilities include conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, documenting progress, acting as a health/medical resource for staff, training staff in the medical arena, conducting audits and reviews of medications, and ensuring company compliance with state requirements and organizational policies regarding medical care and records. The position also involves occasional travel and participation in an on-call rotation for triage of medical concerns for one week per month.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
• Acceptable background check & driving record check
