Licensed Physical Therapist (PT) Part-Time or Full-Time
About The Position
EmpowerMe Wellness is hiring a Licensed Physical Therapist to provide therapy and wellness services to older adults through EmpowerMe 360, an integrated care model that connects therapy, wellness, and medical providers. The therapist helps residents live stronger, safer, and more independent lives by building trusted relationships and supporting their independence, confidence, and satisfaction. The role includes assessments, personalized care planning, evidence-based interventions, care coordination, wellness check-ins, caregiver communication, and telehealth support. The therapist works collaboratively with other providers, upholds professional and ethical standards, and contributes to program development and continuous quality improvement. The position offers predictable scheduling with no late nights or constant driving, and opportunities for leadership, specialty programs, and professional development. Compensation is $52.00–$62.00 per hour.

Responsibilities
- Provide therapy and wellness services to older adults.
