About The Position
EmpowerMe Wellness is hiring a licensed physical therapist to provide therapy and wellness services to older adults in senior living communities. The therapist is part of EmpowerMe 360, an integrated care model connecting therapy, wellness, and medical providers to support residents in living stronger, safer, and more independent lives. The role includes conducting assessments, creating personalized care plans, delivering evidence-based interventions, coordinating care, communicating with caregivers, and supporting residents through wellness check-ins and telehealth. The therapist also contributes to program development and continuous quality improvement while maintaining professional and ethical standards. The position offers predictable scheduling without late nights or constant driving, opportunities for leadership and professional development, and a collaborative, team-focused culture.
