About The Position

EmpowerMe Wellness is seeking a licensed Physical Therapist to provide therapy and wellness services for older adults through its EmpowerMe 360 integrated care model. The role supports residents of senior living communities in living stronger, safer, and more independent lives. The therapist assesses residents, develops personalized care plans, and delivers evidence-based interventions as part of a collaborative team. The position also supports care coordination, wellness check-ins, caregiver communication, and telehealth services, while contributing to program development and continuous quality improvement. This is a PRN role.
