About The Position

EmpowerMe Wellness is seeking a Licensed Physical Therapist to provide therapy and wellness services to older adults through EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers. The therapist helps senior living residents live stronger, safer, and more independently by providing assessments, individualized care plans, and evidence-based interventions as part of a collaborative team. The role also supports care coordination through wellness check-ins, caregiver communication, and telehealth support, and contributes to program development and continuous quality improvement. The position offers predictable schedules without late nights or constant driving, opportunities for leadership and specialty programs, and a supportive, team-focused culture.

Requirements

• Current professional license in the physical therapy discipline (PT); the posting also lists licenses in PTA, OT, COTA, or SLP.
• Passion for empowering seniors to live their best lives.
