About The Position
EmpowerMe Wellness is hiring a Licensed Occupational Therapist to provide therapy and wellness services for older adults through its EmpowerMe 360 integrated care model. The role connects therapy, wellness, and medical providers to support residents in living stronger, safer, and more independent lives. The therapist conducts assessments, develops personalized care plans, and delivers evidence-based interventions as part of a collaborative team. The position also includes care coordination, wellness check-ins, caregiver communication, and telehealth support, along with contributing to program development and continuous quality improvement. The role offers predictable schedules with no late nights or constant driving, as well as opportunities for leadership, specialty programs, and professional development.
Requirements
- Current professional license in the relevant therapy discipline.
