Licensed Clinical Social Worker (LCSW)-PRN
About The Position
Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered neurological rehabilitation in Irvine, California, and surrounding areas. PRN positions are available for clinicians who will accept patients for the duration of their stay. The organization provides treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses in patients’ homes and communities. Clinicians work with interdisciplinary teams to develop integrated treatment plans and provide individualized, one-on-one services. Treatment uses creative activities and an evidence-based approach to address functional goals and support durable outcomes. Patients may be served at any point on the continuum, from severe brain injury to job readiness.
Requirements
- Completion of a Master's degree in Social Work.
- State licensure as an LCSW.
- Excellent planning, organization, writing, and time management skills.
- Ability to communicate with families, professionals, and co-workers.

Responsibilities
- Assess, counsel, and direct medically complex patients.
- Develop individualized treatment plans to assist patients with adjustment to disability, resource linkage, and behavior modification.
