About The Position
Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered neurological rehabilitation in patients’ homes and communities in Clairemont, California, and surrounding areas. PRN positions are available for clinicians who will accept patients for the duration of their stay. The organization provides rehabilitation for traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses. Clinicians work as part of interdisciplinary teams to create integrated treatment plans and deliver individualized, evidence-based services in functional settings. Treatment is provided one-on-one and tailored to each patient’s needs, from severe brain injury through job readiness. Requirements
- Master’s degree in Social Work.
- State licensure as a Licensed Clinical Social Worker (LCSW).
- Excellent planning, organization, writing, and time management skills.
- Ability to communicate with families, professionals, and co-workers.
- Experienced Licensed Clinical Social Worker.
Responsibilities
- Assess, counsel, and direct medically complex patients.
- Develop individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification.
