Licensed Clinical Social Worker (LCSW)-PRN
About The Position
Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered neurological rehabilitation in Beaumont, California, and surrounding areas. PRN clinicians accept patients for the duration of their stay. The organization provides treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses in patients’ homes and communities. Clinicians work with interdisciplinary teams to create integrated treatment plans practiced in home and community settings. The approach is evidence-based, individualized, and designed to address functional goals and support durable outcomes. Services are provided one-on-one, with patients across the continuum from severe brain injury to job readiness.
Requirements
- Master's degree in Social Work.
- State licensure as a Licensed Clinical Social Worker (LCSW).
- Excellent planning, organization, writing, and time management skills.
- Ability to communicate with families, professionals, and co-workers.
Nice To Haves
- Experience as a Licensed Clinical Social Worker.
Responsibilities
- Assess, counsel, and direct medically complex patients.
- Develop individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification.
