Licensed Clinical Social Worker (LCSW)-PRN
About The Position
Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered neurological rehabilitation in patients’ homes and communities in Chino, California, and surrounding areas. PRN positions are available for clinicians who can accept patients for the duration of their stay. The organization treats traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses. Clinicians work with interdisciplinary teams to develop integrated treatment plans and use individualized, evidence-based approaches to address functional goals.
Requirements
- Completion of a Master’s degree in Social Work.
- State licensure as a Licensed Clinical Social Worker (LCSW).
- Excellent planning, organization, writing, and time management skills.
- Ability to communicate with families, professionals, and co-workers.
- Experience as a Licensed Clinical Social Worker.
Responsibilities
- Assess, counsel, and direct medically complex patients.
- Develop individualized treatment plans to assist patients with adjustment to disability, resource linkage, and behavior modification.
