About The Position

Easterseals Florida is seeking a Lead Community Health Resources Navigator to serve children and adults with disabilities in the South Lake County areas (Mascotte, Minneola, Groveland, and 4 Corners). This role involves leading a team in providing health information and advocacy, making referrals to community resources, conducting disability screenings for young children, and recruiting clients. The position requires extensive collaboration with community partners to ensure access to health and disability services. The ideal candidate will be a current or former resident of Lake County, Florida.

Requirements

  • Bachelor's degree in social work, education, or related field and four years of experience.
  • Certification in Health Services (CNA, LPN, etc.) also considered.
  • Experience/knowledge of the South Lake County (Mascotte, Minneola, Groveland and 4 Corners) social service network.
  • Ability to work independently and effectively with minimal on-site supervision.
  • Ability to meet approved driver qualifications to drive a personal vehicle for program-related needs (minimum age of 21 and good driving history).

Nice To Haves

  • Bilingual (Spanish/English) preferred.
  • Experience working with individuals with developmental disabilities a plus.

Responsibilities

  • Coordinate and collaborate with South Lake County's neighborhood services, local community programs, childcare centers, and schools to obtain referrals for children and adults with disabilities and their families.
  • Make referrals to community resources and conduct disability screenings for children ages five and younger.
  • Recruit clients for services.
  • Work extensively with community partners to identify clients and ensure access to needed health and disability services.
  • Collaborate with community partners to coordinate vendors and support the planning and execution of community events.
  • Integrate and participate in community events and committee membership within South Lake County and with community partners.
  • Administer developmental screenings for children ages 0–5 using the Ages & Stages Questionnaire (ASQ).
  • Prepare quarterly and annual reports, develop success stories, and track program data.
  • Provide guidance, mentorship, and ongoing support for the team to foster a collaborative environment and ensure alignment with program goals, deadlines, and quality standards.
  • Participate in the hiring process, including candidate evaluation, provide training, and help track team progress to ensure performance expectations and timelines are consistently met.
