About The Position

The Community Health Resources Navigator works with families with children and adults with disabilities in need of a wide variety of assistance for whole health. The position is responsible for providing health information and advocacy for children/adults and their caregivers, making referrals to community resources, and conducting disability screenings for children ages five and younger. The Community Health Resource Navigator is also responsible for recruiting clients. This position works extensively with community partners to identify clients and to ensure access to needed health and disability services. The qualified candidate will be a current or former resident of Lake County, Florida.

Requirements

  • Current or former resident of Lake County, Florida
  • Bachelor's degree in social work, education, or related field and four years of experience
  • Ability to work independently and effectively with minimal on-site supervision
  • Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)

Nice To Haves

  • Bilingual (Spanish/English) preferred
  • Certification in Health Services (CNA, LPN, etc.) also considered
  • Experience/knowledge of the South Lake County (Mascotte, Minneola, Groveland and 4 Corners) social service network
  • Experience working with individuals with developmental disabilities a plus

Responsibilities

  • Coordinate and collaborate with South Lake County's neighborhood services, as well as local community programs, childcare centers and schools, to obtain referrals for children and adults with disabilities and their families in need of guidance and support to secure health and disability services
  • Successfully integrate and participate in community events and committee membership within South Lake County and with community partner
  • Administer developmental screenings for children ages 0–5 using the Ages & Stages Questionnaire (ASQ) to monitor progress and support early identification of developmental needs.
  • Providing health information and advocacy for children/adults and their caregivers
  • Making referrals to community resources
  • Conducting disability screenings for children ages five and younger
  • Recruiting clients
  • Works extensively with community partners to identify clients and to ensure access to needed health and disability services
