The Food Resource Navigator (FRN) is responsible for helping community members, patients, and their family access and navigate clinical and community services and resources and adopt healthy behaviors related to Fairview's Food is Medicine (FiM) initiative. This FRN will be part of Community Advancement's Community Health Programs, Food is Medicine team. The FRN supports providers and clinical and program team members through an integrated approach to care management and community outreach activities that will promote, maintain and improve individual and community health through food security resource connections. The FRN provides social support and informal counseling, individual and group health education, and advocacy for individuals and community food security needs. Providing home visits and health screenings may be required. The FRN will provide equity focused, relationship-based food resource navigation. Primary focus is patient and/or FiM participant engagement through one-on-one sessions and resource connection, cohort or program specific engagement opportunities, patient / participant communications. The FRN will offer program administration support at care sites across the system offering Food is Medicine programming. They will connect and enroll patients in FiM program that is a best fit for their situation / needs. The FRN delivers culturally responsive follow-up and resources around food and our current program offerings to clinics, providers, and patients. Has a deep understanding of local food systems and can act as a main point of contact to support patients in internal and external food resources and programming.
