About The Position

The Food Resource Navigator (FRN) is responsible for helping community members, patients, and their family access and navigate clinical and community services and resources and adopt healthy behaviors related to Fairview's Food is Medicine (FiM) initiative. This FRN will be part of Community Advancement's Community Health Programs, Food is Medicine team. The FRN supports providers and clinical and program team members through an integrated approach to care management and community outreach activities that will promote, maintain and improve individual and community health through food security resource connections. The FRN provides social support and informal counseling, individual and group health education, and advocacy for individuals and community food security needs. Providing home visits and health screenings may be required. The FRN will provide equity focused, relationship-based food resource navigation. Primary focus is patient and/or FiM participant engagement through one-on-one sessions and resource connection, cohort or program specific engagement opportunities, patient / participant communications. The FRN will offer program administration support at care sites across the system offering Food is Medicine programming. They will connect and enroll patients in FiM program that is a best fit for their situation / needs. The FRN delivers culturally responsive follow-up and resources around food and our current program offerings to clinics, providers, and patients. Has a deep understanding of local food systems and can act as a main point of contact to support patients in internal and external food resources and programming.

Responsibilities

  • Partner with other Community Health Workers, Care Management, interpreter services, and other Fairview departments to provide well-rounded support
  • Promote wellness and food security by providing culturally appropriate health information to providers, patients and/or community members
  • Provide individual social and health care support
  • Refer and link to food security resources through health screenings and healthcare information
  • Conduct health related screenings as assigned
  • Assist patients and community members on resource navigation within assigned geographic area and assist other areas as needed
  • Responsible for performing work at multiple Fairview or community sites
  • Performs administrative support and other responsibilities as needed/assign
  • Increase access to healthy food through culturally competent outreach and enrollment strategies
  • Make referrals and coordinate services related to food security and diet-related health conditions
  • Facilitate continuity of care by providing follow-up
  • Link patients to and inform them of available community food resources
  • Develop relationships and partner with community-based organizations to provide warm hand off referrals
  • Articulate and advocate needs of community and individuals to others
  • Be a spokesperson for patients when they are unable to speak for themselves
  • Involve participants in self and community advocacy
  • Map communities to help locate and support needed services
  • Enhance care quality by aiding communication between health system, patient, and community members to clarify cultural practices
  • Educate patients / community members about how to use the health care and social service systems
  • Educate the health and social service systems about patient / community needs and perspectives
  • Establish better communication processes
  • Attend community events to increase awareness about initiatives, programs, and health topics
  • Supports evaluation including story gathering, reminders, incentives, data collection, and other methods
  • Build, maintain and provide continuous improvement on hunger, diet related chronic condition, and general food referral and navigation processes
  • Stays informed of current community health issues and trends, including evidence-based research, local and statewide needs, health disparities, and cultural competencies to contribute meaningfully to program development
  • Contributes to a culture of improvement and innovative thinking
