About The Position

Heritage Health Network is hiring a Lead Care Manager to support members through its Enhanced Care Management (ECM) program. This hybrid position involves working remotely two days per week and in the field three days per week, building in-person connections with members across Los Angeles County. The role coordinates care, develops personalized care plans, collaborates with healthcare teams and community resources, and helps members address gaps in care.

Requirements

• Experience in care or case management; community-based experience is preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.

Nice To Haves

• Knowledge of Medi-Cal and Enhanced Care Management (ECM).
• Bilingual in English and Spanish.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
