About The Position

Stellar Health’s Nova business bears total cost of care risk on behalf of a growing portfolio of ACO and Payor risk contracts. The Nova Performance function owns aggregate results—including quality, total cost of care, and shared savings—and partners with Provider Success to translate network-wide performance goals into provider-level action. The Lead, ACO Performance serves as the general manager and performance owner for two important Medicare ACO relationships: Stellar’s participation in CMS’s LEAD (Long-term Enhanced ACO Design) Model, the successor to ACO REACH, and its partnership with the affiliated provider network of an academic health system. The role is accountable for performance across both programs and works with internal teams and external ACO partners to set targets, analyze results, guide execution, manage incentives, and address program requirements.

Requirements

At least 4 years of experience in healthcare operations, value-based care, ACO/managed care, or account management in a health-tech or provider services environment.
Track record of owning a book of business or accounts end to end, working autonomously and delivering measurable outcomes with limited day-to-day oversight.
Experience leading executive-facing meetings and communicating performance and risk clearly to internal leaders and external partners.
Strong analytical skills, including fluency in Excel and BI tools, and ability to turn performance data into clear insights and actionable recommendations.
Ability to work independently on ambiguous problems, set short-term priorities, and proactively flag when input is needed on longer-term direction.
Excellent written and verbal communication skills and ability to build trust across internal teams and external stakeholders, from operational staff to health system executives.
Collaborative approach and ability to drive outcomes through influence and partnership rather than direct authority alone.

Nice To Haves

Knowledge of Medicare total cost of care risk models, such as MSSP, ACO REACH, the LEAD Model, or similar.

Responsibilities

Own aggregate quality, total cost of care, and shared savings/risk results across two CMS ACO accounts.
Define benchmarks and success criteria for each ACO with Analytics, Med Econ, and functional leadership, and translate them into operational targets.
Own agendas, materials, and facilitation for ACO steering committee and operating reviews, and build an internal executive reporting cadence.
Analyze quality, claims, and total cost of care data to identify performance drivers and develop actionable recommendations.
Partner with Provider Success counterparts to ensure each identified performance gap has a data-driven action plan that is executed and tracked to completion.
Recommend and refine the SVU menu and incentive design for each ACO to optimize outcomes for Stellar, the ACO, and its providers.
Stay current on LEAD Model requirements and milestones, including benchmarking, risk track elections, and reporting obligations, and coordinate internally to meet them.
Proactively identify risks to ACO performance and escalate them to the VP, Nova Performance with a reasoned plan.
Share best practices and lessons learned across the Nova Performance team and help develop playbooks and tooling to scale performance management.
