About The Position

The Kinesiologist / Exercise Specialist is an integral member of the outpatient Diabetes Management Centre and Centre for Complex Diabetes Care, providing evidence‑based exercise assessment, prescription, and education to adults living with Type 1 diabetes, Type 2 diabetes, and diabetes in pregnancy. Working within a hospital‑based ambulatory care setting, this role supports patients in improving glycemic control, physical function, and long‑term engagement in physical activity. The successful candidate will deliver both individual and group‑based interventions, facilitate exercise programming, contribute to patient education resources, and collaborate closely with an interprofessional health care team to support comprehensive diabetes management.

Requirements

  • Bachelor of Science (BSc) in Kinesiology required
  • Current registration and member in good standing with the College of Kinesiologists of Ontario required
  • Minimum 2–3 years of experience providing exercise counselling and education in individual and group settings
  • Strong knowledge of exercise physiology principles as they relate to diabetes management
  • Demonstrated experience in prescribing and progressing structured, goal‑oriented exercise programs
  • Excellent patient assessment, documentation, and program planning skills
  • Knowledge of adult learning principles and behaviour‑change strategies, including Motivational Interviewing and stages of change
  • Demonstrated ability to work independently in a self‑directed manner, including effective time management, prioritization of clinical responsibilities, and sound clinical judgment within an outpatient hospital setting
  • Demonstrated accountability to program and service agreement deliverables, including patient volumes, education sessions, group exercise programming, documentation, reporting requirements, and participation in program evaluation and quality improvement activities

Nice To Haves

  • Master’s degree in Kinesiology or Exercise Physiology considered an asset
  • Experience working with adults with complex or chronic medical conditions in a hospital or health care setting preferred
  • Current Exercise Specialist certification with the American College of Sports Medicine (ACSM) or Canadian Society for Exercise Physiology (CSEP) considered an asset
  • Experience delivering individual and group counselling/education in both in‑person and virtual care environments preferred

Responsibilities

  • Conduct comprehensive assessments of patients’ physical activity levels, exercise tolerance, functional capacity, and readiness for behaviour change, incorporating relevant medical history and clinical considerations.
  • Develop, prescribe, and safely progress individualized, goal‑oriented exercise programs that support diabetes self‑management, weight management, and overall health outcomes.
  • Provide individual and group education on physical activity as a therapeutic intervention for patients with Type 1 diabetes, Type 2 diabetes, and diabetes in pregnancy.
  • Develop, coordinate, and facilitate group exercise classes within the outpatient diabetes program in accordance with hospital policies and best practices.
  • Monitor and evaluate patient progress, attitudes, and behaviours related to physical activity, and modify exercise plans as clinically indicated.
  • Collaborate as part of an interprofessional health care team, serving as a resource to other health care providers on exercise prescription and physical activity guidelines for diabetes.
  • Develop, adapt, and maintain patient education materials and exercise resources that align with evidence‑based practice and program standards.
  • Participate in program planning, quality improvement initiatives, and evaluation activities to support ongoing service excellence and patient outcomes.
  • Deliver care in both in‑person and virtual formats, as required.
