About The Position

The Job Coach / Transition Specialist provides individuals with support and guidance in employment, including developing job skills, finding suitable work, and navigating workplace dynamics. The role is based at the City CPV Hub and works in community settings.

Requirements

• High school diploma.
• One year of experience in a work setting training others.
• Valid New York State driver’s license with an approved record.
• Reliable transportation.

Nice To Haves

• Previous experience with direct care service.

Responsibilities

• Instruct and assist individuals in developing social skills, work skills, and work habits in a community-based setting.
• Orient individuals to new work settings, including work routines, production demands, and quality standards.
• Monitor the quantity and quality of work produced by individuals at a community business site to ensure conformance with the customer’s contract standards.
• Transport or escort individuals to and from community work sites as needed and/or provide mobility training.
