Interim Housing Case Manager

About The Position

This position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.

Requirements

  • Bachelor's Degree in Social Work or related field.
  • 2-3 years of case management experience
  • Knowledge of issues surrounding homelessness, mental illness and substance use
  • proficiency in Microsoft office software
  • good oral and written communication skills
  • good organizational skills
  • conflict management and crisis intervention skills
  • demonstrate clear and professional boundaries
  • act in a professional manner.

Nice To Haves

  • Master's preferred.
  • LCSW preferred

Responsibilities

  • Assess participant physical, emotional, and financial needs through intake and on-going individual meetings.
  • Utilize motivational interviewing techniques to determine participants' readiness to change and plan appropriate interventions
  • Assist participants with prioritizing self-identified goals and planning to achieve those goals.
  • Evaluate progress related to goals monthly
  • Provide necessary resources to participants.
  • Provide support and guidance to participants through process of goal-achievement.
  • Document and collect housing data for reports.
  • Refer participants to appropriate permanent housing.
  • Explore and update permanent housing options for program participants.
  • Assist participants in moving into permanent housing.
  • Make appropriate referrals for services based on participant goals and needs.
  • Work collaboratively with the Residential Manager and staff regarding participant concerns via staff meetings, program log, and written updates.
  • Provide crisis intervention when necessary.
  • Assist with tracking housing retention after participants have exited the Teresa's program.
  • Prepare and maintain current tenant files.
  • Enter all services and required information into HMIS and other service and outcome tracking databases as required.
  • Work collaboratively with the Program Data Manager to ensure data quality.
  • Maintain required professional knowledge and job skills.
  • Attend and participate in staff development opportunities.
  • Represent Deborah's Place mission and values to participants, visitors and others.
  • Perform other duties as assigned.
