About The Position

The EIH Program is a housing-focused, low-barrier and harm reduction program designed to serve adults experiencing homelessness. The EIH program is a referral-only based program that provides each participant with access to a private unit with restroom, shower, and furnishings. The EIH provides onsite comprehensive support services with the goal of assisting all program participants transition from EIH into permanent housing. The Case Manager will work in partnership with program participants to develop individualized housing plans that include personal goals the participant may have in their journey to obtaining permanent housing. Case managers will offer on-going, regularly scheduled case management sessions as well as convenient drop-in options. Case management sessions include assisting the participant in identifying and obtaining the resources and services required to meet their housing goals. For those individuals already assigned a case manager through other providers, the EIH case manager will supplement and support the service plan developed by the other agency and ensure coordination to avoid duplication of services and aid in the transition to permanent housing.

Requirements

  • 1 year paid or volunteer experience in social work or related human services required
  • Proficient in Microsoft Office suite
  • Valid CA driver’s license and insurance, ability to be added onto agency fleet insurance (required)

Nice To Haves

  • Bachelor of Arts in Social Work (BASW) or related human services degree preferred, not required
  • Bilingual proficiency in Spanish preferred, but not required

Responsibilities

  • Maintain a caseload of 20-30 participants.
  • Provide comprehensive case management services. This may include: Initial eligibility verification and enrollment, Ongoing needs and status assessments, Budgeting, Information and referral, Crisis de-escalation, Safety planning, risk assessment, and mandated reporting, Employment and benefits assistance, Advocacy, Other direct service provision.
  • Meet with participants at a frequency and location determined by their needs.
  • Engage in the consistent application of evidence-based practices.
  • Participate in case conferencing and collaborative care coordination.
  • Maintain compliance with contract, agency, and licensing requirements.
  • Achieve identified program and position deliverables.
  • Adhere to task completion deadlines.
  • Maintain thorough and accurate documentation of participant services in accordance with agency and contractual standards.
  • Attend all job-related meetings, including program staff meetings and agency-wide meetings.
  • Participate in opportunities for learning and skill maintenance/development.
  • Utilize supervision appropriately, maintaining open lines of communication and activity.
  • Assist with other duties as assigned.
