About The Position

Alcott, a leading nonprofit agency dedicated to providing vital mental health and supportive housing services to low-income adults across Los Angeles County, is seeking a compassionate and clinically skilled Clinical Case Manager – Interim Housing to join our team in Watts, CA. Requirements

• A Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or a related mental health field is required.
• Active BBS registration as an ASW, AMFT, or APCC is required within 90 days of hire.
• Strong crisis intervention, assessment, and documentation skills are required.
• Excellent written, verbal, and organizational skills are essential.
• Local travel between agency sites is required.
• A valid California Driver’s License, clean driving record, and insurability under agency policy are required.
• Successful completion of Live Scan background check is required.
• COVID-19 vaccination and at least one booster required (medical or religious exemptions must be documented upon hire).

Nice To Haves

• Experience providing clinical services in substance use treatment, corrections, interim housing, or community mental health settings is preferred.
• Military-experienced candidates are encouraged to apply.
• Familiarity with harm reduction, trauma-informed care, and motivational interviewing is highly desirable.
• Bilingual Spanish/English candidates are strongly encouraged to apply.

Responsibilities

• Provide professional, evidence-based clinical services to adults residing in Alcott’s Interim Housing program, supporting their transition from correctional facilities into community living.
• Conduct comprehensive intake assessments and develop individualized, client-centered treatment plans.
• Facilitate individual counseling and psychoeducational group sessions.
• Monitor client progress and adjust treatment plans to support measurable recovery goals.
• Deliver evidence-based interventions grounded in harm reduction and trauma-informed care.
• Provide 1:1 counseling to strengthen coping skills and readiness for change.
• Respond to crisis situations within housing sites and community settings.
• Coordinate referrals to medical, mental health, and substance use treatment providers.
• Connect clients to community resources and recovery supports.
• Collaborate with multidisciplinary teams to ensure integrated care.
• Maintain accurate and timely documentation in the electronic health record.
• Participate in case conferences and staff meetings.
• Ensure compliance with agency standards and funder requirements.
