About The Position

Merakey is seeking an Intensive Case Manager for Adults to join their team in Carlisle, PA. This role involves traveling throughout Cumberland and Perry Counties to provide Intensive Case Management (ICM) services to adults with serious mental health needs. Responsibilities include conducting assessments, developing service plans, providing ongoing support through linking, monitoring, advocacy, and skill-building, and assisting clients in accessing psychiatric care, community resources, and social supports. The position also involves educating families and significant others, maintaining accurate documentation, participating in clinical supervision and staff meetings, and being on-call for their caseload.

Requirements

• Bachelor’s degree in sociology, social work, psychology, or a related field; OR high school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years’ experience in human services with one year in direct client contact; OR a registered nurse (RN) license.
• Valid driver's license and reliable transportation required.

Responsibilities

• Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
• Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
• Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
• Support clients in accessing psychiatric care, community resources, and social supports.
• Provide education and support to families and significant others.
• Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
• Participate in clinical supervision, staff meetings, and quality improvement initiatives.
• On-call for caseload.
